Basic principles of the combined and united use of therapeutic physical factors
BASIC PRINCIPLES OF THE COMBINED AND UNITED USE OF THERAPEUTIC PHYSICAL FACTORS
Today the successive prescription of two or several therapeutic factors during the course of treatment is understood by the combined physical therapy. The procedures are applied in the following way: one by one in one day; in different days; the course influence of one factor changes another course of treatment. Combination is the simultaneous use of two or several therapeutic factors.
There are difficult physicochemical and physiological processes in the basis of mechanism of action of complex physiotherapy, here the phenomena of synergism, sensitization and antagonism are of great importance.
The principle of synergism is revealed through the increase of final effect due to including physical factors of unidirectional action in the medical complex (arrangement of alike reactions from the side of certain physiological systems or potentiation of influence of different physical factors).
Sensitization occurs in those cases, when one of the ways of physical therapy brings an organism or other its systems in the state of hypersensitization to influence of another physical factor.
The principle of antagonism often finds its practical use to reduce the unwanted ways of actions of one or two factors: for example, douches and baths of contrasting temperatures allow to prevent atony of vessels and venous hypotension that can occur under the influence of a thermal factor.
The principle of aggravation of local reaction is especially evident when combining factors with mainly systemic and local action. It is determined that the local reaction is aggravated, if the general effect (baths, douches etc.) is exerted after the local procedure.
In properly selected physiotherapy complex the positive effects of operating in one direction several physical factors are summed up, the negative effect of certain components is weakened, an impact on different systems of an organism and sides of the pathological process is exerted, the period of aftereffect of jointly applied procedures increases.
The example of synergistic action of heat and galvanic current are the four-chamber baths and electromud procedures (“galvanic mud”). But such a combination of two strong stimuli should be used only in moderate dosing, which are adequate to the functional condition of organism of patients. The method of electrotherapy with the simultaneous use of several factors is inductothermoelectrophoresis.
Compatibility and sequence of physiotherapeutic procedures. Many physio- and balneoprocedures cause considerable and lingering reaction of the patient. That is why one should not prescribe several intensively effective procedures in a day. When summarizing the influence of strong stimuli, different functional abnormalities of the nervous, cardiovascular and other systems of the organism can appear.
Medical complex should be prescribed individually, taking into account the form and stage of disease, peculiarities of clinical presentation, presence of concomitant diseases, age of a patient, the mechanism of action of physical factors and their compatibility. But complex therapy can not be provided as polypragmasy because the prescription of a big number of procedures can be an excessive load for an organism and cause the aggravation of pathological disorders.
In the majority of cases it is reasonable to limit oneself to one of such procedures as medical electrophoresis, inductothermy, the UHF field effect in thermal dosings, DMW-, CMW-therapy, ultrasound, ultraviolet irradiationin in erythermal doses, mud, ozocerite and paraffin applications, hydrogen sulfide and radon baths in a day.
The prescription of another electrotherapeutic procedure in one day when using medical electrophoresis or other types of electrotherapy is possible if it is applied to a limited area of the body, for example, to accessory sinuses of nose, or it supplements the first one according to its action: for example, the effect of the electromagnetic waves and ultrasound with subsequent electrophoresis.
The procedures for one reflexogenic area, through which the general influence on the organism is exerted, are not compatible in one day (collar and sinocarotid zone, mucous coat of nose etc.).
Procedures, opposite according to their action, are usually incompatible (heat and cold, soothing and stimulant) except those cases when it is necessary to achieve a contrast reaction (for example, douches or baths of contrasting temperatures) for the purpose of tempering an organism.
When planning complex therapy and solving practical questions of compatibility and incompatibility of physioprocedures it is necessary to consider the peculiarities of action of certain physical factors.
Medical electrophoresis is a combined effect of the galvanic current on the human organism and injected with its help medicinal preparations. Various pulse currents (rectangular, dynamic etc.) and also sinusoidal modulated currents in rectified mode and fluctuating currents can be used for electrophoresis. In the basis of mechanism of action of this physiotherapy method the reflex phenomena are of great importance. They are developed with the participation of various parts of central nervous system in the result of irritation of peripheral nerve receptors with the current and electrically charged particles of medicinal preparations. Under the influence of galvanic current the medicinal preparations, injected into an organism in other ways, are accumulated in the tissues and organs, located in a big quantity. This principle is used at the so-called intratissular or intraorganic electrophoresis. Reasonability of simultaneous bipolar introduction of two medicinal preparations: one from the anode and another from the cathode, was determined during some diseases. According to the bipolar technique, medicinal preparations that have different polarity, are simultaneously injected from both the collar electrode and from the electrode, located on the lumbar area. Strengthening of the therapeutic effect at the bipolar electrophoresis according to this technique has been noted: calcium and bromine (by neurasthenia and neurasthenic state with vegetative disorders), magnesium and iodine, novocaine and iodine (by cardial and cerebral atherosclerosis). One should consider the ability of medicinal preparations to stay for a long time (“to be deposited”) in the skin after the electrophoresis. To avoid the extraction of a medicinal preparation from the “skin depot”, one can not place the cathodic electrode on the areas of the skin, where medicines from the anode (for example, calcium and magnesium) were injected with the previous procedure, and vice versa, one can not place the anode in the area of the previous location of the cathode. Thermal procedures (warming with “Soluks” lamp, mud, paraffin and ozocerite applications), as well as the use of other physical factors, such as the UHF, electromagnetic waves and ultrasound fields, favour the lowering of the skin polarization resistance to the flowing of the galvanic current and the increase of the quantity of medicines introduction by electrophoresis. Maximal increase of electrophoretic penetration of skin after the ultrasound procedure of average power is observed during the first 30 minutes and lasts for 3 hours, and after the microwave irradiation in slight and medium doses – in 1-3 hours and remains heightened for 6-12 hours. Administration of HF-electrotherapy and paraffin applications with the subsequent electrophoresis of medicinal preparations has a greater therapeutic effect than each of these factors separately. Clinical results, achieved by the effect of the UHF field or DMW with the subsequent electrophoresis in complex treatment of the chronic nonspecific bronchopulmonary diseases are positive by the infectious inflammatory and traumatic diseases of peripheral nervous system. Ultra- and microwave physical factors after the electrophoresis are used much rarely. The procedures in such sequence can be prescribed for the purpose of fast washout of a medicinal preparation from the “skin depot” and its accelerated penetration into blood through strengthening of blood and lymph exchange.
Electrosleep therapy, dynamic, interference and sinusoidal modulated currents are compatible in one day with other electrotherapy procedures if there are indications for combination. It is not recommended to use the mentioned physical factors at the same day in complex with the electrical procedures of systemic action (galvanization and Vermel medical electrophoresis, general franklinization etc.).
High-, ultrahigh-frequency and microwave factors are: inductothermy, UHF electrical field in thermal dosing, CMW- and DMW-therapy are incompatible with each other and also with darsonvalization, erythermal ultraviolet irradiationin, gas, hydrogen sulfide, radon baths, douches of high pressure with low and contrast temperature and thermal-medical procedures.
Darsonvalization (local) is incompatible with inductothermy, UHF-, CMW- and DMW-therapy, ultrasound, erythermal ultraviolet irradiationin, mud therapy, radon, hydrogen sulfide and carbonic acid gas baths.
Magnetotherapy is incompatible in one day with other electrotherapy procedures of common effect, UHF- and microwave therapy, inductothermy and local ultraviolet irradiation. If there are indications, magnetotherapy can be combined with the medical electrophoresis, electrosleep, diadynamic and amplipulse therapy, laser therapy, ultrasound and phonophoresis and also with barotherapy (during the treatment of peripheral vessels diseases). The effect is exerted at once, or at first alternating magnetic field and then electrophoresis or phonophoresis of medicinal preparations are applied. The increasing of the skin penetration for a medicinal preparation and the increasing of general effectiveness of treatment is achieved by this combination. The course the low-frequency magnetic field effect on the area of inferior cervical and superior thoracic segments 30-60 min. before bicycle training favours the optimization of the therapeutic effect of patients with CHD, who had myocardial infarction. Inclusion of magnetotherapy and pelotherapy into the treatment and rehabilitation complex improves the efficiency of rehabilitation of the contingent of patients with limbs fractures.
Ultraviolet irradiations influence the metabolism processes, blood pressure, vascular tone, central and peripheral nervous system. Small and medium biodoses of ultraviolet irradiation stimulate the function of sympathoadrenal system and host defenses. General ultraviolet irradiations in suberythermal doses are compatible practically with all physiotherapeutic procedures. Ultraviolet irradiations in erythermal doses are incompatible with electrophoresis on the same area, inductothermy, microwaves, darsonvalization, ultrasound, massage, roentgenotherapy, thermal medical and water procedures. After the ultraviolet erythema roentgenotherapy can be provided only in a week. After the course of ultraviolet irradiation another its course can be prescribed not earlier than in 1 month. By irradiation in erythermal doses the evidence of photochemical reactions depends on a set of conditions. The ultraviolet erythema manifestation is accelerating and its degree is increasing with the previous effect on the correspondent skin areas with the UHF field, galvanic current, red and infrared rays and after local darsonvalization. Red and infrared rays irradiation, which is provided simultaneously or after the ultraviolet irradiation, reduces the evidence of skin reaction. During the prescription of preventive and therapeutic ultraviolet irradiations one should consider that sensitivity to ultraviolet rays is volatile during the whole year: it is higher in spring and in winter and lower in summer and in autumn. One should also bear in mind that the organism’s sensitivity to ultraviolet irradiation increases when applying aminazine, iodine, sulfanilamid, arsenic, mercury and other preparations. In connection with this it is not reasonable to prescribe general ultraviolet irradiations simultaneously with electrophoresis of aminazine, novocaine and vitamin B1, as these medicinal preparations themselves can cause allergic skin reaction, which increases by the additional effect of ultraviolet irradiation of persons, who have contact with photosensitizers: methylene blue, dichlorobenzene, creosote, tar and others. At the same time introduction of calcium, dimedrol, insulin and sodium thiosulfate into the organism lowers sensitivity of the organism to ultraviolet irradiation.
Laser irradiation. The main principles of compatibility and incompatibility of laser therapy with other physioprocedures are the same as for other types of phototherapy. Magnetotherapy in combination with laser radiation (“magnetic-laser therapy”) reduces time of healing of the infected wounds and trophic ulcers as compared with other methods of treatment. Lazerpuncture in combination with its balneomud treatment increases the efficiency of treatment of patients with degenerative-dystrophic diseases of joints and spine. It is recommended to prescribe balneomud treatment in those days, when lazerpuncture is not provided, although their combination during one day is possible.
Thermal medical procedure (mud, paraffin, ozocerite applications), when prescribing them on reflexogenic zones or large areas of the body, are incompatible with water procedures (baths, douches), microwave therapy, UHF in thermal dosings, general franklinization, light baths, infrared irradiation, erythermal ultraviolet irradiation, ultrasound and other physioprocedures of systemic action. Mud treatment is not compatible with other types of thermotherapy.
Mud applications (mostly local) are compatible with the receiving of galvanization and medical electrophoresis, franklinization, electrosleep, dynamic and sinusoidal modulated currents, magnetotherapy, ultraviolet irradiation in one day, but it is desirable that the interval between these procedures is at least 2 hours (except for special techniques – electrophoresis of mud, inductothermoelectrophoresis etc.). Local mud procedures of a small area can be compatible in one day with balneological procedures and solar radiation.
It is reasonable to apply air baths before peloid procedure; mechanotherapy, exercise therapy and different types of electrostimulation of neuromuscular apparatus should be applied directly after the mud application.
Douches of high pressure, low and contrast temperature (stream, circular, contrast and others), bathing in a pool, gas, mineral (sodium chloride, sea, iodine-bromine), aromatic (coniferous, sage, turpentine), hydrogen sulphide and radon baths are not compatible with thermal medical procedures (inductotherapy, microwaves, light baths and infrared radiation).
General sea, pearl, oxygen baths, bathing in a pool according to individual indexes can be prescribed in one day: microwave therapy, ultrasound with the interval of at least 2 hours. It is reasonable that the ultrasound is applied before the bath, and microwaves – after it. When combining hydropathic procedures with phototherapy, at first irradiation and then hydrotherapy is provided.Acupuncture is more effective in combination with climatotherapy when there are pulmonary and cardiovascular diseases. Acupuncture can be combined in the medical complex with electrotherapy, balneotherapy, massage and exercise therapy according to indexes, especially when there are concomitant diseases.
Inhalation of medicinal preparations (aerosol therapy, electroaerosol therapy) is compatible in one day with electrotherapy and other physiotherapeutic procedures. Different antibiotics and other antibacterial preparations, bronchial spasmolytic and vasodilator, antiinflammatory and hyposensitization agents are introduced with inhalation. Inhalation can be prescribed both before and after the medical electrophoresis or effect of HF, UHF and microwave physical factors, ultraviolet irradiation and massage. When there are chronic nonspecific lung diseases, it is more reasonable to apply the procedures in the following order: chest massage, inhalation therapy, exercise therapy, climatic procedures, methods of apparatus physiotherapy; the following sequence of procedures is also effective: inductothermy (or effect of UHF or CMW), inhalations, exercise therapy, climatotherapy. When electroaerosols are provided, the joint effect of medicinal preparations and electric charges of micro-drops on the organism takes place. Availability of an electric charge provides high dispersion and stability of aerosol particles that penetrate through bronchi to the level of alveoli. Medicinal effect of medical aerosols is conditioned not only by their influence on the mucous membranes of respiratory tracts, but also by the rapid absorption of aerosol particles and their accumulation in the system of lesser circulation. It is recommended to make 30-60 minutes’ break between the procedures of electroaerosol therapy and other procedures.
General scheme of the rules of compatibility of therapeutic physical factors is given in the table.
Combined methods of apparatus physiotherapy and balneophysiotherapy. The peculiarity of the combined methods of apparatus physiotherapy is the increase of efficiency through synergistic action orientation of the used factors:
Electrophoresis of medicinal preparations with constant and pulse currents (see corresponding part above). Preparations that have synergistic action orientation with the used current and mode of the impact are mainly recommended (adrenaline, ascorbic acid, gangleronum, iodine, calcium, magnesium, nicotinic acid, novocaine, papaverine, no-spa, pilocarpine, proserin, lydasum, tripsin, aminophylline).
Electrosleep-electrophoresis. It is recommend to use of the preparations, which affect the higher nervous activity, vascular tone and blood flow and which have the resorbing action: aminazine, analginum, haloperidol, hydrocortisone, lithium, no-spa, oxybate sodium, seduxen, theophylline, phrenolon, elenium and others. The parameters of electrosleep are dosed (current strength, pulse frequency, duration of a procedure). The use of additional constant component is recommend.
Vacuum-phonophoresis of medicinal preparations. Mud preparations (peloidinum, mud expression), aminophylline and others are used. The technique is local, segmental, on the pain areas and irradiation. Special vacuum cups, in which pressure of 40-50 kPa (300-400 mm Hg), are used, preferably without forming of a bruise. Intensity is 0,2-0,4 W/cm2, pulse mode - 10 ms.
Magnetic-laser therapy is a medical method, when the magnetic field and laser radiation simultaneously affect the necessary zones of the patient. Peculiarities of the effect: therapeutic effect of these two factors is combined, which leads to greater depth (30%) of penetration of laser energy and its utilization. Advantages: it reduces time of the procedure and recovery period.
Magnetic-ultrasound therapy is a medical method, when the magnetic field and ultrasound simultaneously affect the necessary zones of the patient. Peculiarities of the effect: therapeutic effect of these two factors is combined, which leads to more effective treatment, fast decrease of edema, stronger possibility of drugs to pass the histohematogenous barriers to the focus while using drugs simultaneously (for example, ultrasound chemotherapy + magnetic ultrasound on the focus in lungs). Advantages: it considerably reduces time of treatment of the majority of diseases.

Photochemotherapy. Combined effect of long-wave ultraviolet radiation. Technique: special ointment – 0.1% 8-MOS, 0.1% of povalen, 0,1-1%ofmethoxalen is applied with a thin layer without rubbing on the source of injury (by psoriasis, Vidal's disease, alopecia) in one hour before the procedure. Remains of the ointment are removed with a cotton tampon before the procedure. Irradiation is carried out with the intensity of 0.5 to 4-5 biodoses, 15-25 effects in a course, every other day.
Galvanic inductothermy. The combination of direct current and inductothermy. Technique is the simultaneous effect of direct current and magnetic field on one zone or the effect of magnetic field on the domain of passage of the field lines of electric current. It is recommend to use electrodes of conductive rubber and graphitized tissue. At first the apparatus for inductothermy and then the apparatus for galvanization is switched on in 1-2 minutes. Turning off is held in a reverse order. Lower ranges of power value and current density are recommended. Duration of the procedure is 10-20 minutes, every other day, 10-15 procedures in a course. The electrophoresis of inductothermy is carried out according to the same technique in compliance with the rules of electrophoresis.The combined procedures of pulse currents with inductothermy are carried out as the diadynamic inductothermy, diadynamophoresis inductothermy, amplipulse inductothermy and amplipulsephoresis inductothermy. Dosing is carried out according to the parameters of pulse currents and inductothermy.
Hydroelectric baths is a hydropathic procedure, by which a patient, who is in the bath, is affected by the simultaneous effect of the temperature factor of water and galvanic current. The basic mechanisms of specific and stimulating action, spare capacities of sympathetic-adrenal system and carbohydrate metabolism are increasing as a result of physical and chemical processes and a change in gene juncture, quantitative and qualitative correlation of ions in tissues. Peculiarities of the effect: spasmolytic, sedative, desensitizing, analgetic and vasodilatory.
Indications: diseases of the musculoskeletal system, diseases of peripheral nervous system, functional disorders of the central nervous system; disorder of the sleep formula; coronary heart disease; phenomena of not pronounced atherosclerosis.
Contraindications: common for physiobalneotherapy, electrical current intolerance.
Hydromagnetic baths is a hydropathic procedure, by which a patient, who is in the bath, is affected by constant magnetic field. The effect of water temperature and magnetic field from 4 sides of the bath has an evident sedative or tonic, spasmolytic and hypotensive action. When the direction of the artificial magnetic field in the bath and natural field of the Earth (vector of magnetic field of the Earth passes from the North to the South) match, tonic effect is produced on the organism, the circulation of natural magnetic fields of the organism, of the so-called internal magnetic field (I.Z. Samosiuk and co-author, 2000), is increasing. When the opposite effect takes place, the sedative effect is observed. Peculiarities of the effect: the temperature factor and constant magnetic field from 4 sides affect the patient, who is in the bath with water: head and right side of the body – to the north, legs and left side – to the south. Position of the bath should be directed to the Earth poles (south-north).
Indications: CHD, exertional angina of the 1st-2nd functional class; atherosclerosis of coronary vessels of the I degree; cerebrosclerosis, essential hypertension of the 1st-2nd stage; obliterating diseases of limbs vessels; Raynaud's disease, Parkinson’s disease, diseases of peripheral nervous system, diseases of the musculoskeletal system.
Contraindications: common for hydropathic procedures, essential hypotension, inclination to bleeding, general infirmity, dementia.
Magnetohydrolaser vacuum massage (MHLVM) is a method of combined application of apparatus physio- and balneotherapy: magnetic field, laser radiation, vacuum massage and water. Mechanism of the procedure consists of the effect of all these factors on the organism of the patient.
Combination of therapeutic physical factors and medicamental agents.
In complex therapy therapeutic physical factors are often used together with therapeutic agents. Medical electrophoresis, phonophoresis and inhalations belong to physical and pharmacological methods. The mentioned methods, which allow to inject medicines into the organism, have certain advantages and specificity of action. Complex therapy of many diseases includs, except for physiotherapy, the introduction of medicinal preparations in other ways (by mouth, parenterally etc.).
Due to the change of possibility of penetration of histochemical barriers and adsorption-trophic function of tissues under the influence of physical factors, accumulation of drugs increases in organs and tissues not only in the area of localization of the effect, but also in the zones, connected with it in a reflex way. Factors with greater penetration ability affect the penetration of medicinal preparations into the internal organs: microwaves, magnetic fields and electric currents. The mentioned physical factors potentiate the effect of pharmacological agents – local anaesthetic, psychotropic, antiinflammatory, spasmolytic, antibacterial preparations; agents that affect clotting.
The intensified accumulation of medicines, which circulate in the organism, in the skin and mucous membranes takes place when many physical factors are applied according to the techniques of the general influence and hydropathic procedures.
During physiotherapy of cardiovascular and other diseases one should bear in mind, that ultrasound, centimeter and decimeter microwaves and magnetic fields in small doses (20 mT) possess more evident hypocoagulation action and potentize the anticoagulants.
Administration of electrosleep and medications can be appropriate during the diseases of CNS (cerebrolysinum, trental, nootropil, solcoseryl etc.). When electrosleep and above mentioned preparations are applied, their better passing through blood-brain barrier and penetration into the pathological focus are observed.
Such action is observed by the central electroanalgesia and other variants of electrocranial physiotherapy (electric field of UHF, DMW, SMC-therapy etc.).
